Advance Directives

Definition: A legal document that describes medical decisions you would make for yourself if you are unable to do so for any reason. An advance directive can also designate someone to speak on your behalf to make medical decisions for you. Examples of these decisions include whether or not you would wish to be resuscitated in the event of a cardiac arrest, whether you would choose mechanical ventilation if you are unable to breathe on your own, and whether you would want artificial methods used to feed and hydrate you if you are unable to eat or drink fluids on your own.

Examples:
Jim's oncologist recommended he fill out an advance directive, so his family would be aware of his wishes for further medical care if he was unable to express them himself.
